Instructions: To complete the Supplier/Contractor Questionnaire;
- Be sure you have your corporate Dun & Bradstreet number, which is required
to complete the first page of the questionnaire. Go to the
Dun & Bradstreet Web Site
to get a number, if needed.
- Your E-Mail address is required.
- At the bottom of the first page, click on 'Go to Part 2 - Enter Commodities'
to continue with the questionnaire.
- You MUST enter at least one commodity code in Part 2.
- After entering commodities in Part 2, click on the "Submit" button at the
top of the form.
- On the last screen, you may enter your work site preferences and any comments
you may have.
