This report is used to assist the Department of Energy in
analyzing work force composition and fluctuation, and provides a tool in evaluating the
subcontractors performance in diversity.
Report employees filling positions identified as full-time
status or part-time status on the Subcontractors approved staffing plan. Employees
filling those positions who were transitioned or assigned from any other source (company
transfer or new hire) should be counted. Employees filling intermittent or temporary
positions on the Subcontractors approved staffing plan should not be counted.
Part-time refers to an employees status (i.e., employee works less
than 40 hours per week), intermittent refers to the fact that the position is filled
by your company less than 1000 hours per year and has been exempted from workforce
transition on the staffing plan.
Quarterly
Reporting periods end on March 31, June 30, September 30 and December 31.
Subcontractors must submit the report by the 10th
day following the close of each reporting period. This report should be submitted via

- Contractor/Facility Total - Enter the total number of
employees as of the last calendar day of the reporting quarter as described above under
"Coverage."
- Black (not of Hispanic origin) For each job
category line item (Appendix A),
enter the total number of male and female employees having origins in any of the Black
racial groups of Africa as of the last calendar day of the reporting quarter.

- Hispanic For each job category line item (Appendix A),
enter the total number of Hispanic male and female employees of Mexican, Puerto Rican,
Cuban, Central, South American, or other Spanish culture or origin, regardless of race, as
of the last calendar day of the reporting quarter.

- American Indian/Alaskan Native For each job
category line item (Appendix A),
enter the total number of male and female employees having origins in any of the people of
North American and who maintain cultural identification through tribal affiliation or
community recognition as of the last calendar day of the reporting quarter.

- Asian/Pacific Islander - For each job category line
item (Appendix A),
enter the total number of male and female employees having origins in any of the original
people of the Far East, Southeast Asia, the Indian Subcontinent, or the Pacific Islands as
of the last calendar day of the reporting quarter. (This area includes for example, China,
Japan, Korea, the Philippine Islands, and Samoa.)

- Other (not of Hispanic origin) For each job
category line item (Appendix A),
enter the total number of male and female employees having origins in any of the original
peoples of Europe, North Africa, or the Middle East as of the last calendar day of the
reporting quarter.
