To assist the Department of Energy with assessment
of salary and residential information.
Report employees filling positions identified
as full-time status or part-time status on the Subcontractor's approved staffing
plan. Employees filling those positions who were transitioned or assigned from
any other source (company transfer or new hire) should be counted. Employees
filling intermittent or temporary positions on the Subcontractor's approved
staffing plan should not be counted. Part-time refers to an employee's
status (i.e., employee works less than 40 hours per week), intermittent
refers to the fact that the position is filled by your company
less than 1000 hours per year and has been exempted from workforce transition
on the staffing plan.
Annual
(CY) - Reporting period ends December
31.
Subcontractors must submit the report by the
10th day following the close of each reporting period. This report
should be submitted via email or fax to:
- Location of Residence - Break
down residences by (a) county (b) zip code, and (c) city. Some cities
will have more than one zip code and these should be listed separately.

- Total Employees - All employees
filling positions identified as full-time status or part-time status on
the Subcontractor's approved staffing plan. Employees filling those positions
who were transitioned or assigned from any other source (company transfer
or new hire) should be counted. Employees filling intermittent or temporary
positions on the Subcontractor's approved staffing plan should not be
counted.

- Total Annual Salaries & Wages (Gross
wages reportable on W-2 Form.) - ACTUAL taxable compensation to include
base/premium pay, incentive compensation, and other special pay. Include
non-bargaining and bargaining unit personnel performing work on this contract.
Do not include out-of-payroll benefits, such as savings plans, retirement,
etc.
