RFP Amendments:

        RFP Amendment 1 -- 10/31/2008   (WinZip required.)

        Changes:

1. Exhibit C, Form A-1, was revised to include CY 2010 for Base Year 2 in the Labor category. Also, revised Pay Item 7 from $500,000 to $1,500,000 in the Other Costs category.
2. Exhibit C, Proposal Response Form – Revised to read one-hundred, eighty (180).
3. Instructions & Information to OFFERORS, Section 15, revised to read one-hundred, eighty (180).
4. Instructions & Information to Offerors, Section 20: The physical address is Highway 58, Blair Rd. Bldg. 1007, MS 7596, Oak Ridge TN 37830.
5. Instructions & Information to Offerors, Section 21 includes the requirement to submit Exhibit C, Form C-1 with the Technical Volume. Exhibit C, Form C-1 is now included.
6. Exhibit C Cover Letter added the Form C-1 – LOWER TIER EXPERIENCE STATEMENT to the Table Of Content page.
7. The Org Chart, Fig 1 in the Misc folder is removed from the RFP.
8. Exhibit B, Section SC-2 COMMENCEMENT, PROSECUTION, AND COMPLETION OF THE WORK, Part E – language was revised.

        RFP Amendment 2 -- 10/31/2008   (WinZip required.)

        Changes:

1. Exhibit C, Form A-1, was revised to add the labor category Project Manager (WFT) line item 1.02 and adjust the remaining line items in the Labor section of this form. Also, addd footnote no. 2 to this form.
2. Exhibit C, Form A-2, Pay Item No. 6 - Management Support Team was revised to replace the phrase "Pay Item 3.0" to "Pay Item 1.0".
3. Provided procedure BJC-EH-5110, Biological Monitoring for Industrial Chemicals.
4. Revise Exhibit C, Form Q - Statement 4 to include the value of "five" in the statement "number of years or are now in progress for the following owners or prime contractors.”
5. Instructions & Information to OFFERORS, Position and Qualifications Section for Project Manager - revised the first line of required qualifications to read: "-15 or more years of applicable experience, including supervision of RCTs and/or IHTs."
   Exhibit D revised qualifications for the Project Manager has been revised to reflect the same change as in Instructions & Information to OFFERORS.
6. Exhibit B, Special Conditions - SC-11 titled "Contractor-Furnished Government-Owned Materials and Equipment" is hereby revised to include a listing of IT equipment (Attachment 8) that will be provided.
7. Exhibit J, Wage Determination - SCA Wage Determination has been updated for this procurement with Wage Determination No. 2005-2493 dated 06/09/2008 and Wage Determination No. 1994-0520 dated 05/29/2008.
